Before completing this form, students must apply for admittance in the specific certificate program here. Students must also have completed all required certificate coursework. Do not submit this form until after your admission start date, even if courses for the certificate have been completed.
The University will confirm the student's status and mail the certificate to the address of record. Students must complete each course with a grade of "B" or better and an overall GPA of 3.0 or above to be awarded the certificate.
Certificate earners are awarded a digital badge for each certificate they complete. Badges can be digitally displayed anywhere to recognize attaining a new level of knowledge.
